What is YAG Laser Capsulotomy?

YAG laser capsulotomy, also known as posterior capsulotomy, is a common procedure used to treat posterior capsule opacification (PCO), which is a frequent complication following cataract surgery. During cataract surgery, the natural lens of the eye is replaced by an intraocular lens (IOL), but sometimes the posterior capsule can become cloudy, impairing vision. YAG laser treatment is used to create a small opening in the clouded capsule to restore clear vision. This procedure is quick, non-invasive, and performed with minimal discomfort, making it an ideal solution for treating this condition.

YAG laser is used to target the posterior capsule, delivering precise laser energy to make an opening in the clouded tissue. This allows light to pass through again for clear vision.

Posterior Capsulotomy is reported to occur in 20%–40% of patients 2–5 years after surgery [1].
This procedure is typically recommended when patients experience vision impairment due to PCO, such as blurry vision or glare. If left untreated, PCO can affect daily activities, such as reading or driving.

To perform the procedure, the physicians uses a focused laser beam to create an opening in the posterior capsule, restoring the normal pathway of light to the retina. This laser procedure is quick, often taking only a few minutes, and is typically performed in an outpatient setting. The use of eye drops ensures that the procedure is painless, and recovery time is minimal, with most patients noticing improved vision within hours.

During a conventional YAG laser capsulotomy, the eye care professionnal carefully targets the posterior capsule using the laser to create a small, clear opening. This restores the passage of light, improving the patient’s vision.

  1. Preparation: The patient’s eye is numbed using local eye drops to ensure no pain during the procedure.
  2. Laser Application: The YAG laser delivers a precise pulse of laser light in the vitreous (posterior offsets < 500 microns). The delivered energy causes growth and decay of a rapidly forming cavitation bubble. The cavitation bubble generates a mechanical wave which propagates toward the posterior capsule with enough force to break tissue.
  3. Post-Procedure Care: Patients typically experience improved vision within a few hours. It’s important to follow the ophthalmologist’s instructions carefully for aftercare to avoid complications. Follow-up visits are recommended to monitor any potential issues, such as increased pressure in your eye or retinal detachment.

How PROCAP™ Works?

With conventional capsulotomies performed with conventional YAG offsets < 500 microns, the cavitation bubble does not complete its natural life cycle. The mechanical wave disrupts the posterior capsule and the bubble collapses without any further involvement.

With PROcap, capsulotomy is performed with a 2mm YAG offset. The greater distance between the laser strike and the IOL (2 mm) causes the cavitation bubble to behave differently. It can complete its natural life cycle of growth and decay and when it collapses, it implodes asymmetrically, propelling a high-speed liquid jet from the bubble towards the capsule. 

It is this liquid jet, rather than the propagating mechanical wave, that causes the disruption, and, because of the distance involved, the IOL is completely safe from harmful pitting.
